Which Appointment Do You Need?

Choose the option that best describes you so we can guide you to the right evaluation.

New to Discovering Wellness?

Or has it been more than 12 months since your last evaluation?

You’ll need a comprehensive evaluation that includes in-depth testing, a full review of your health, and a personalized wellness protocol based on your current needs.

Returning Within 12 Months?

Welcome back!

Your follow-up appointment is a focused 30-minute session to review your progress, address changes, and refine your protocol as needed.

The 12-Month Guideline: If your last comprehensive evaluation was more than 12 months ago, we require you to schedule a new evaluation to ensure your protocol reflects your current health.

How do I know which appointment to schedule?

New clients and clients whose last comprehensive evaluation was more than 12 months ago should schedule a new evaluation. Returning clients seen within the previous 12 months may schedule an appropriate follow-up service. Contact the clinic if you are uncertain which appointment is right for you.

How should I prepare for my appointment?

Preparation varies by service. You may receive instructions related to fasting, hydration, hair-sample collection, medications, supplements, or dietary records when the appointment is scheduled.

What should I bring to my appointment?

Bring completed patient forms, a current list of medications and supplements, relevant laboratory results, and any questions you would like to discuss.

Why is a new evaluation required after 12 months?

Health history, medications, supplements, lifestyle, symptoms, and wellness priorities can change significantly over time. A new evaluation allows the practitioner to reassess your current needs before making updated recommendations.
